New Professional Services Health Care Claim Form Coming April 1, 2014

Starting on April 1, 2014, all health care providers that participate in the Texas Workers’ Compensation system must be prepared to bill paper claims on the revised CMS-1500 Claim Form (version 02/12). Texas Workers’ Compensation insurance carriers and their agents must also be prepared to receive and process the new CMS-1500 (version 02/12).
