County mutuals in the top 80 percent of the Texas auto insurance market must submit information for TDI´s auto rate guides. For each of the four driver profiles, provide the median figure for minimum liability coverage for 12 months. For example, if there are nine possible rates for Driver A for a given territory, provide the one that is in the middle of the range, or the fifth rate. If there are an even number of possible rates for a driver profile, use the lower of the two middle rates. Do not include discounts. Provide rates in effect on June 1, 2001.

| NAIC Group and Company Number | The NAIC three-digit group and five-digit company numbers. |
| Consumer Information Telephone Number | The number consumers may call to get information about rates or how to get a quote. If your company has a toll-free number for consumers, please use that number. |
| Company Contact for Additional Information | The name of the person responsible for reporting the information on this form to TDI. |
| Company Contact Telephone Number | The direct phone number of the person listed as the company contact. |
| Company Contact E-Mail Address | The e-mail address of the person listed as the company contact. |
| Driver A | Median Annual Rate for Minimum Liability for Driver A: male age 25-64 or female age 21-64 with no at-fault accidents or major traffic convictions who drives to and from work. BI+ PD (20/40/15), 1 year, no discounts |
| Driver B | Median Annual Rate for Minimum Liability for Driver B: single male under age 21 with no at-fault accidents or major traffic convictions. BI+ PD (20/40/15), 1 year, no discounts |
| Driver C | Median Annual Rate for Minimum Liability for Driver C: single female under age 21 with no at-fault accidents or major traffic convictions. BI+ PD (20/40/15), 1 year, no discounts |
| Driver D | Median Annual Rate for Minimum Liability for Driver D: male or female over age 64 with no at-fault accidents or major traffic convictions who drives for pleasure and errands only. BI+ PD (20/40/15), 1 year, no discounts |
