What to expect on the online renewal form
You will need to affirm knowledge of applicable rules and laws.
The Independent Review Organization (IRO) applicant must affirm:
- They are familiar and compliant with the Texas statutes and rules for independent review in Texas. These include:
- Texas Insurance Code Chapter 4202.
- 28 Texas Administrative Code (TAC) Part 1, Chapter 12, and to rules of the Division of Workers’ Compensation, including TAC Part 2, Section 133.308.
- They are not a subsidiary of, or in any way owned or controlled by, a payor or a trade or professional association of payors. Any violation of this prohibition will result in forfeiture of the IRO certification. Other administrative penalties are possible.
You will need to attach the supplemental certifications.
If needed, the IRO applicant can use the Supplemental Certification for IRO Renewal (Form LHL007) to satisfy the exhibit requirements for:
- Certification of no material change (28 TAC 12.108).
- Certification of primary office (28 TAC 12.103 (13)(A)-(E)).
