Rules and Procedures for Requesting a DVD Loan

General Information

  • The Resource Center DVD Loan Request Form must be used. New customers must fill out a paper form and fax it, mail it or e-mail a scan. Customers who have checked out and returned materials from the TDI Resource Center may use the shorter Online DVD Loan Request Form.

  • One form must be filled out for each requested Show Date. (A Show Date may extend from one to seven days in duration. Extensions of seven days may be requested and are always granted unless the DVD is needed for another customer.) Longer Show Dates may be granted on a case-by-case basis.

  • Up to three titles can be selected for each Show Date. A title includes both Spanish and English versions if both are desired. (You do have to indicate that you want both Spanish and English titles on the form.) More than three titles may be granted to customers in good standing on a case-by-case basis.

  • Three late returns will result in suspension of privileges.

  • In the event of loss, the requester agrees by signing the DVD Loan Request Form to replace the DVD program promptly. Failure to do so will result in collection efforts and/or referral to the Office of the Attorney General as appropriate.

  • Copyright regulations prohibit any duplication or tampering of the DVDs. Anyone violating the regulations may face a misdemeanor charge. Your completed Request form is your agreement not to copy the program. Violations of Federal Copyright laws will result in the immediate suspension of borrowing privileges and prosecution by the copyright owner can result in financial penalties and/or jail.


  • Forms must be received in the Resource Center at least 21 days prior to the Show Date. Since titles are booked on a first come, first serve basis, early requests will help ensure title availability. Exception: Austin area patrons are requested to give up to 10 days notice.

  • Requests may be faxed to (512) 804-4621, scanned and e-mailed to, or mailed to TDI-DWC Resource Center, 7551 Metro Center Drive Ste. 100 MS 25, Austin, TX 78744-1645.

Returning Videos

  • DVDs must be placed in the correct cases. All materials must be cushioned properly against damage.

  • The Audiovisual Usage Form which accompanies Resource Center materials must be filled out completely.

  • The Due Date shown on the Audiovisual Usage Form is the date the program is due back in the Resource Center. Returns must be made by overnight or two-day courier service, e.g. UPS, Federal Express.

  • Materials must be insured for $500 per DVD, unless otherwise noted. (Replacement value on some DVDs will be higher.)
