How to Apply for Inclusion in the Registry
Private providers of vocational rehabilitation services may apply to be listed in the Texas Department of Insurance - Division of Workers' Compensation (TDI-DWC) Registry of Private Providers of Vocational Rehabilitation Services, provided below.
28 Texas Administrative Code (TAC) §136.2 requires a private provider who wishes to be included in the TDI-DWC's registry to complete, sign and submit an agency-approved registration form. Section 136.2(b)(5) requires a statement showing that the private provider is credentialed as an LPC, CCM, CRC, CVE or CDMS.
Although only individual private providers can be included in the registry, an individual provider may list a business affiliation if desired. The application is valid for a one-year period.
