Notice of New Form (TWCC-151, Attorney Online Application Form)

To expedite and improve the process for attorneys to submit requests for approval of attorney fees, the Commission has developed and implemented the Web-enabled Attorney Fee Processing System (WAFPS). WAFPS will allow attorneys to submit requests for attorney fees online in lieu of filing a TWCC-152, Application for Attorney's Fees.

The Commission has adopted the TWCC-151 for attorneys to apply to use the WAFPS and receive an Access Code. In addition, since the WAFPS does not provide for a signature with each request, the TWCC-151 provides a one-time signature that applies to all online requests by an attorney.

Attached is a copy of the new TWCC-151, Attorney Online Application Form to be used in conjunction with WAFPS. A paper copy of the TWCC-151 should be filed with Customer Service for assignment of an access code. Please mail the hard copy of the TWCC-151 with an original signature to Texas Workers' Compensation Commission, Customer Services, MS-602, 4000 South IH-35, Austin, Texas 79704-7491.

An electronic version of the TWCC-151 may be downloaded from the Commission's website www.tdi.texas.gov.

The Commission encourages immediate use of this new form for applying to use WAFPS.
