Requestors must identify the quarter(s) and service type(s) requested. Additional information regarding these data sets can be found in the following PDF documents:

   A hard copy of the items listed on the Publications Price List can be requested:

In person at the Texas Department of Insurance, Division of Workers’ Compensation (DWC) Mailroom, located at 7551 Metro Center Drive, Suite 100, Austin, Texas 78744.

By mail by:

  • printing out the “Publication Price List”
  • filling out the “Publication Price List” for the requested items; and
  • sending the request [Publications Price List] with a check or money order for the total amount to:
