Licensing of Life Settlement Providers, Brokers, and Life Agents acting as Life Settlement Brokers
The Company Licensing & Registration Office handles the following transactions related to the Certificate of Authority (license) of entitites engaging in the business of Life Settlements:
- Initial issuance of Certificate of Authority (COA) for Life Settlement Providers, Brokers, and Life Agents acting as Life Settlement Brokers
- Two-year renewals of COA for Life Settlement Providers, Brokers, and Life Agents acting as Life Settlement Brokers
- Surrender and/or Nonrenewal of Certificate for Life Settlement Providers, Brokers, and Life Agents acting as Life Settlement Brokers
- Change of Information for Life Settlement Providers, Brokers, and Life Agents acting as Life Settlement Brokers

- FAST Pass Form (for electronic fingerprint appointment)
- Fingerprint Card Scan Authorization Form
- Application for Viatical or Life Settlement Registration or Change of Information (Form FIN430). Contact the Company Licensing and Registration Office to have this form e-mailed to you. NOTE: Effective September 1, 2011, House Bill 2277 imposed new statutory requirements on the business of life settlements. If you wish to apply for a TEMPORARY license, contact us. There is no fee associated with this temporary license.
